Cafeteria Plan -- Voluntary Payroll Deduction Vendors
The following companies are voluntary payroll deduction vendors that have qualifying products that can be included in the State of Missouri Cafeteria Plan for 2009. In accordance with 1CSR10-15.010 these are the only voluntary payroll vendors that may solicit business on State property for their listed products.
If you have a current payroll deduction for one of these products, you will automatically receive the pretax benefits of the Cafeteria Plan in 2009 unless you actively "Opt Out" by selecting the Cancel Pre-Tax box on the Enrollment form. The MOCafe plan year is January 1 through December 31st. Open Enrollment is held from October 1 through December 1 each year. Cafeteria Plan requirements can be found in the Plan Summary document located at www.mocafe.com.
A new employee must actively "Opt Out" of the Cafeteria Plan within 31 days of his/her hire date to not participate in MOCafe.
Coverage for mid-year new-hire enrollments may be effective on the first day of the month following approval of the enrollment form.
You may also make future changes to your account within 60 days of any qualifying change in status event.
